SURVE ("we," "us") provides non-medical home care and wellness services in Broward County, Florida. This policy explains what information we collect, how we use it, and your choices.

Information we collect. Information you give us when you call, text, email, or fill out a form: your name, phone number, email address, home address and zip code, and details needed to schedule and provide your visits. We also collect basic website usage data (pages visited, browser type) through cookies.

How we use it. To schedule, confirm, reschedule, and cancel visits; to communicate with you about your care and your account; to answer your questions; to operate and improve our services; and to meet legal obligations.

Text messages (SMS). If you agree to receive text messages from SURVE (for example, by saying yes when our receptionist asks if we can text your appointment confirmation, or by texting START to (234) 444-5402), we will send texts about your appointments: confirmations, changes, and cancellations, plus replies to your questions. Message frequency varies. Message and data rates may apply. Reply STOP at any time to opt out, or HELP for help. Consent to receive texts is not a condition of receiving our services.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Sharing. We do not sell your personal information. We share it only with service providers who help us operate (scheduling, communications, and payment processing) under confidentiality obligations, when required by law, or to protect safety.

Security. We use reasonable administrative, technical, and physical safeguards to protect your information.

Your choices. You can ask to access, correct, or delete your information, or opt out of texts, by contacting us. Reply STOP to any text to stop messages.

Children. Our website is not directed to children under 13.
