How you sign up
- By phone: when you call SURVE at 954-333-8886 to book, change, or cancel a visit, our receptionist asks: "Is this the best number to text your appointment confirmation to?" We only text you if you say yes, and we record your answer.
- Online: check the box "Yes, SURVE may text me about my appointments at the number above. Msg frequency varies. Msg & data rates may apply. Reply STOP to opt out, HELP for help." on our contact form at https://survehealth.com/contact. The box is never pre-checked.
- By text: text START to (234) 444-5402.
What we send: appointment confirmations, changes, and cancellations, and replies to your scheduling questions. No marketing.
